Liability Terms for AboutFace-USA® Veteran Ride Service Program

Release of Liability

Please read carefully! This is a legal document that affects your legal rights.

The undersigned individual (the “Rider”) desires to obtain facilitated curb-to-curb automobile transportation through AboutFace-USA®, a Georgia non-profit corporation, in collaboration with Common Courtesy, Inc., and accordingly executes this Release of Liability (this “Release”). In consideration of the services performed by AboutFace-USA® and Common Courtesy, Inc., the Rider hereby freely and voluntarily executes this Release under the terms below:

  1. Release: Rider does hereby release, discharge, and covenant not to sue AboutFace-USA® and Common Courtesy, Inc., their members, officers, directors, employees, volunteers, agents, successors, and assigns (collectively the “Organization”) from and against any and all liability, claims, or demands that arise or may hereafter arise directly or indirectly from the transportation services (the “Services”) provided by the Organization. This release includes but is not limited to any and all liability or claim that the Rider may have against the Organization with respect to bodily injury, personal injury, illness, death, or property damage that may result from the Services, whether caused by the negligence of the Organization or otherwise.
  2. Waiver of Rights; Legally Binding: Rider understands that by executing this Release, he/she is waiving legal rights including the right to sue, and intends to be legally bound by this Release.
  3. Photo Release: Rider consents to and authorizes the use of photographs, videography, or digital recordings of them taken by the Organization for any purpose, including but not limited to promotional materials, social media, and other media created by the Organization.
  4. Memory Loss: If the Rider has shown signs, symptoms, or been diagnosed with any level of memory loss, they agree to travel with a companion at all times while using the Services.

By participating in the AboutFace-USA® Veteran Ride Service Program, you acknowledge and agree to the terms outlined in this Release of Liability. This program is designed to ensure the safety and well-being of all participants, and compliance with these terms is mandatory for all riders.
